Does anyone have information about this patch? I have been trying to research this and find nothing matching.

I do believe it was somehow related to the ARISS program with the schools. This is the 5" patch with ESA, ISS, and STS-121 up and STS-116 down. Thomas Reiter was the ESA astronaut on those flights via the ISS.

I always seek the mission behind the patch and if possible the design meaning of the patch.

I believe it is simply a rejected proposal for a name and an insignia for what ultimately became the Astrolab mission.

Erik is correct: For what Detlev van Ravenwaay told me there have been some more designs for that mission which were rejected by ESA.
