How about an astronaut who didn't fly at all, like Yvonne Cagle, who was assigned to "Management Astronauts."

Yvonne Cagle and Fernando Caldeiro are two big mysteries as to why they never flew.

Some astronauts (like Gus Loria, and John Bull from the Apollo era) were disqualified for physical or medical reasons. Caldeiro eventually died from brain cancer, but I believe that diagnosis came down after he had transferred from the Astronaut Office to the Aircraft Operations Branch.
