If you don't mind printing and ring binding from a pdf file, the official name is JSC-17327 Flight Data File, Medical Checklist (later renumbered JSC-48031). There were over 10 revisions of the generic version of this checklist over the history of the program. Each individual flight has its own Flight Supplement that contained procedures for the on-orbit DSO's (Detailed Supplementary Objectives). They had the same JSC numbers, but were followed by a dash and the number of the flight. Certain missions with dedicated middeck medical experiments also had a flight specific Biomedical Checklist with procedures for these devices. | |
