What would happen if an International Space Station crew person was unable to make a flight? Would the entire crew be replaced by the backup crew, or would just the back up crew person be promoted to the prime crew?

Also take this hypothetical scenario: if Chris Hadfield had been unable to fly TMA-07M would the entire back-up crew of Yurchikhin, Nyberg and Parmitano have flown or would Parmitano simply have replaced Hadfield? Also if Hadfield was scheduled to later command Expedition 35 would that now fall to Parmitano?

Other options are (1) if the entire crew were replaced Yurchikhin would command Expedition 35 or (2) if just Hadfield/Parmitano had swapped Romanenko would have taken over command of Expedition 35 as the more experienced person?
