I was reading this morning that NASA had considered moving the ISS to avoid a lump of debris due to pass about 3 km from it. This leads me to ask - how many manned spacecraft have been hit by either debris or meteorites?

I believe the first recorded case was Faith 7 which had an impact crater on its window which was put down to meteorite damage.
