I just read that Frank Borman turned down a chance to walk on the moon. Michael Collins also turned down a chance to command Apollo 17 and walk.
Did any other astronauts turn down the opportunity to train and walk on the moon?

Didn’t Gene Cernan turn down the lunar module pilot assignment on Apollo 16? He wanted a command of his own.

In some ways this can be also said about Schirra. If he had a dedication to continue with the program, he would probaby demonstrate a different attitude during AS-204/205 (and, as a consequence, on Apollo-7, too). This could possibly get him another mission - per rotation he could probably be on Apollo-13 or so.
