I am looking for information concerning the trainings in the Apollo simulators. I'm interested in...

CSM simulator:

  • How many failures and combinations thereof were used during training to prepare the astronauts for their journey?
  • How many flights in the simulator have been successful?
  • How many of them were failures?
LM simulator:
  • How many failures and combinations thereof were used during training to prepare the astronauts for their landing on the moon and return?
  • How many landings to and starts from the moon in the simulator have been successful?
  • How many of them were failures?
General questions:
  • Who developed the malfunctions and the possible breakdowns?
  • Are there books or websites on the topic?
