I am not sure your production numbers are correct, According to Chris Spain that is the amount sold in 2005 and 2006 of what he believed to be production over run.
Since they were never available to the general public they are extremely scarce, although examples from what seems to be a production overrun came to market in 2005/6. By my count there were roughly 150 each of the RE and HS patches sold at this time, along with about 60 EC versions.
These patches come up for sale quite often.

Has it been confirmed that 150 were given to each of the Apollo 17 crewmen?

If there were 150 patches made for each of the Apollo 17 astronauts then there must be about 300 for each in circulation. As the 2005/2006 sales would account for almost all of the ones given to the astronauts and we know that was not the source of those patches.
Do not get me wrong, these are rare and awesome patches (I have all three). It is just the total number quoted that I question. I have never seen a production number for these patches and I have no idea if one in fact exists?

Goodness knows when or where I heard this but I seem to recall a minimum run of 300 was the norm. That would tally with the 150 each plus Chris's estimate of the "overrun".
