I just watched a program explaining a defective wire mesh (tank diffuser) used in the space shuttle external tanks to equalize pressure during ascent depletion. Where did this gas come from, what was it, and how is this an issue for Space Launch System?

Liquid hydrogen or liquid oxygen from the ET warmed by the engines into gas is used to pressurized the ET as the propellants are depleted.

SLS does the same thing.
