I've seen many instances of flown flight plan or checklist pages that have areas where data or updates from Mission Control should be noted, but with these areas remaining blank. I understand that sometimes updates are not required, but in the case of this checklist page from the current RR Auction (Item 3176), it seems that the Hi Gain Antenna pitch and yaw data should be noted.

Does the lack of notations on pages that appear to have required them concern other collectors even though they are "certified" as being flown?

There were two LM Activation Checklists carried on board so it is probable that the other checklist recorded the angles if any notation was made or were other than as indicated in brackets on the page.

The National Archives at Fort Worth have photocopies of the flown checklists which would establish if there were any notations made.

I had a quick look through the TEC transcription and could see no reference to angles as Buzz went through the activation steps at the stage covered by the page.

As to the question of notations made by the crew, I would prefer a page that had actual information or notations recorded by the crew but of course many of the pages were just procedures/information and to acquire any flown page would be great, especially from the astronauts own collection.

Even better would be the complete checklist.

Unless Mike Collins wrote the figures on the walls of the spacecraft.
