Does anyone know when the Apollo 1 crew patch was first designed and approved?

The artwork (and by association, the patch) was approved by George Mueller in June 1966. Just one week before the fire, however, George Low wrote to Deke Slayton that:
...since then, it has been decided that the mission will most likely not be called Apollo 1. The approval of the patch is therefore temporarily withdrawn, and it may have to be changed after a firm designation for the mission is obtained. This is expected to occur next week at the earliest. No further release showing this patch should be made.

Does that mean they were wearing the artwork before approval or were all of the photos of the crew wearing the patch taken in that one week?

No. The patches were manufactured in August 1966, and had been used for the crew's pressure suits since. Low's memo just kept the door open if the "Apollo 1" would need to be altered (revised patches would have been made).
