I know someone here will know the answer or be able to link me to the original thread.

I'm looking for information about the Fallen Heroes patch. Who designed it, who makes it, etc.

I have one of those. I either got it from The Astronomy Store, which I think has gone away, or from Spaceflight Now's store. This February 2002 article: http://www.spaceflightnow.com/news/n0202/08galaxy/ , includes an ad for the patch, although the link is dead. Perhaps they can help you.
