How many of the target of opportunity charts were flown on each lunar mission?

I think that this Apollo 15 chart was one of four? As the moon rotated, I guess ground tracks and lighting changed so you might need a few of these massive charts.

It was normally one of four charts, but on Apollo 15, it was one of five charts.
