I've been collecting Apollo era Life magazines (I'm scanning the Apollo related pages if people want pdf copies) and was recently able to obtain the Apollo 11 'To the moon and back' special edition published on August 11 1969.

It was sold as including an 'original supplement', which turns out to be an orthographic projection chart of the Moon drawn by Antonin Rukl. This is it.

There is no information on the chart as to its origin or date. However...

The chart shows the locations of all the Apollo landings, so obviously is not from the original publication. It refers specifically to '6 landings', so is not including planned Apollo sites.

The Life magazine has no copyright dates apart from 1969, and has the original $1.50 price tag. It is, as far as I can tell, identical to the digital copy on Google books and was sold as an original copy.

What I have been unable to verify is whether the landing special was reproduced at a later date and a supplement included, or whether the 'supplement' has been included by mistake as it has passed through different owners. I've also not been able to find another image online like the map I now have (which I do like, and may frame!)

Any ideas as to the origin of the map?

I have a copy of this magazine bought new by myself and can confirm the supplement you linked was not included in the publication.

Same here. I have 2 copies of the Apollo 11 Life special edition. One was purchased in person at a drug store in 1969. The other was given to me in a plastic sleeve by an elderly gentleman who was downsizing and getting rid of a large collection of period newspapers, magazines, and books covering space exploration and conjecture since the 1930s, and so I assume it is also an original copy.

Neither included anything like the described lunar map. Doubt it was included with the original magazine, unless it was distributed only to subscribers. I don't even know if the special edition was sent to subscribers or if they too had to purchase it at the news stand like everybody else.

My copy purchased for me by my mother in 1969 defintely had no map included. If it had, I would have remembered, and I would still have it!

Yep, I agree... I have never seen this before and certainly NOT in the mentioned magazine.
