This photo shows activity around the aft fuselage access hatch of shuttle Columbia. This was the location of a pad accident during the countdown test for STS-1 on March 19, 1981. Rockwell workers John Bjornstad and Forrest Cole entered the aft section for an inspection and were overcome by the 100% nitrogen atmosphere. They died as a result of oxygen deprivation.

When was the worm logo taken off Columbia's payload bay doors? Following the first OMDP after STS 9/Spacelab 1 in 1983?

The last Columbia flight to have the NASA "worm" was STS-93 in July 1999.
