I recently heard an extract from Chris Hadfield's new book. It recounted how he had trouble with his eyes due to not removing enough of the cleaning fluid from his visor that prevents fogging.
Was this ever an issue with other flights, especially the moon landings?

The design of the helmets is such that air is directed across the inside surface of the visor to prevent misting. That is why on occassion I have seen footage of the visor misting momentarily as the astronauts speak and then clear almost immediately.
That said, I am sure on a particulary arduos EVA it could become an issue.
