I need to compile a list of every error or mistake in the Apollo flights, after Apollo 1, no matter how small. I'm starting my research on a fiction book where I need to know every little goof up, glitch and mistake that was made from liftoff to splashdown on all of the Apollo missions after Apollo 7.

I've spent a lot of time pouring through books and online, looking for the info but haven't found a good comprehensive list. Any assistance would be greatly appreciated.

Have you looked in the back sections of the NASA Mission Reports by Apogee for each specific mission? They have a discrepancy report that may be a good starting point for info as it discusses the discrepancies for the CSM, LM, Launch Vehicle, etc., by mission.

There also is the collection of "Apollo Experience Reports" on the NASA Technical Information server. I think they're all publicly available. The ones I've gone through (they're done by system) typically have by-mission anomalies, which aren't necessarly "mistakes" made by people, though.

And they are also featured on The Apollo Lunar Surface Journal, as is the crew tech. debriefing.

Is there any place online or any one book that would list anything that went wrong on each manned Apollo mission that flew? I'm researching info for a fictional book where each error in the entire program will be addressed...

I have, but I haven't been able to find ANY list of anomalies for all the missions. I've searched the mentions above and I can't find any list that goes over the errors on each mission. If someone knows where in these reports to look, I'd be grateful as I don't think I have several months to go through hundreds of thousands of pages of data to pick out each individual thing that was messed up.

If there's one book out there which might have this data, I'd be fine with paying for it, I just can't justify buying stacks of books on each mission, and again, going through every line trying to find the proverbial needle in the haystack...

I don't believe a single list like you describe exists.

Rather, as earlier mentioned, the post-flight technical debriefs, discrepancy and later experience reports, captured that information in the format needed at the time.

There is an internal NASA infographic, Significant Incidents and Close Calls in Human Spaceflight, but it encompasses more than just the Apollo program.

Not one single source (within one volume) but you may find it helpful to refer to the NASA Mission Director's Summary Report for each mission. I believe all can be found online, for instance on the Apollo Lunar Surface Journal/Apollo Flight Journal.

For example, in the Apollo 14 MDSR:

TABLE 9: Launch Vehicle Discrepancy Summary
TABLE 10: CSM 110 Discrepancy Summary
TABLE 11: Lunar Module 8 Discrepancy Summary
TABLE 12: Apollo 14 Crew/Experiment Equipment Discrepancy Summary.

There was a post a few years back, someone asked the question "Why was there no S-III stage in the Saturn V"? I had told part of the story.
