I'd never seen one of these before so I snagged it on eBay; a 1972 Lunar Sample Assignment receipt from a prominent Cornell Chemistry professor.
As best I can tell from NASA's Lunar Sample Catalog website, the 64501,17 number indicates this was part of a loose soil sample from Apollo 16.
Am I interpreting things correctly? Are these sample receipt pages unusual to run across?

I had it on my eBay watch list and rather wish I'd snagged it. I'd never seen one before, and it's an interesting, specific, Apollo-related primary source document. Good buy!
