Luckily - looks like no-one injured - otherwise it could have been even more serious.

Another example of procedures not being carried out and short cuts being taken by the tone of the news report ?

I just love how they call it an anomaly.
Lockheed had another anomaly with a 1.6m primary telescope mirror a while back.
