Today was released the Soyuz MS-19 crew patch via Anton Shkaplerov's Twitter and Instagram:
Friends, there are less than two months left before the launch of the Soyuz MS-19 spacecraft! The emblem is developed by Roscosmos together with Channel One, the Yellow, Black and White Studio.

The composition of the emblem is built around the image of the ascending Soyuz launch vehicle with the Soyuz MS spacecraft. The mission name, Soyuz MS-19, is located on both sides of the rocket.

The large yellow star symbolizes the ISS [International Space Station] and the three white stars represent the crew of three cosmonauts: Anton Shkaplerov, Klim Shipenko and Yulia Peresild — all of them will become the stars of the first feature film shot in space. The excitement of the flight of non-professional space flight participants is reflected in the contour of the cardiogram that symbolizes the beating of their hearts. In the lower field, there is the planet wrapped with film to symbolize the shooting of a film. Its name, "The Challenge," is inscribed in the frame of the stylized film.

On the top of the emblem border, there are the names of the expedition members, and on the bottom — the flag of the Russia. The right field contains the logo of Roscosmos.

The excitement of the flight of non-professional space flight participants is reflected in the contour of the cardiogram that symbolizes the beating of their hearts.
Given what has been released about the plot of "The Challenge," this detail on the patch seems to have a double meaning.
...the plot involves a cosmonaut who suffers a cardiac arrest during a spacewalk and, although he survives, he will require surgery to ensure he can handle the Soyuz return to Earth. A female cardiac surgeon, named Zhenya, has to be sent to the ISS to perform the procedure with only a few weeks to prepare for the trip.

Will this be the official patch, or is it a promotional patch based on the launch of two out of the three cosmonauts?

Two of the three are strictly there for the film.

I think given that Shkaplerov was the one to debut it, this is the Soyuz MS-19 patch.

If there is an alternate patch for this mission, I would expect it to be one with the character names to be used in filming the movie.

500 patches were produced in Moscow as a souvenir for VIPs at tbe Baikonur Cosmodrome. Ordered by Roscosmos.

The prime and backup crew patches, as well as apparel with the mission insignia and "Challenge" movie merchandise is now available directly from Roscosmos through its commercial division Glavkosomos' online shop.

(The patches are priced at 990 rubles each, about $14, but shipping to the U.S. is very expensive, about $87.)

In March, Shkaplerov will land with Debrov and Vande Hei. Maybe Roscosmos will do a landing version of the MS-19 patch for space suits?

25 patches for the main crew and 25 for backups are produced without plastic for Sokol space suits and onboard suits and delivered to Zvezda and Kentavr. It is for every Soyuz crew.

I asked a question to Shkaplerov. He answered they have not changed patches on Sokols yet.

The word in the lower field of the landing patch means the spacecraft callname "Astraeus" or what? Any more information?

Vande Hei and Dubrov were wearing the MS-18 patch, and it appears Shkaplerov was wearing the normal MS-19 patch. Photo below is a crop from their photo just before hatch closure.

As mentioned above, the bird on Pereslid's embroidery is the logo/mascot of the Galchonok charity, which works to provide treatment for children with central nervous system disorders. Peresild was a founding member of the fund and the insignia was designed by the organization for her flight.
