It is Fedor's personal patch which was on his spacesuit during his EVA 43 together with Ryazanskiy.
I know it's already the third version of it with different slogans. But also on the previous versions we can see a boy looking at the sky — Fedor in his childhood.
The boy is standing on the geographic point where is situated Batumi — home town of Fedor. And this time Fedor is a grown up person-cosmonaut. And his hand is still on Batumi's geografic point!

The big polar star is representing the ISS. Three stars on the planet are situated in the geographic points of Moscow (Koroliov, Batumi and Baikonur). The four previous flights of Fedor are represented by four sails of the ship and the fifth one, by the long red flag.
And we also can see on the ship two letters in Russian - FY (Fedor Yurchikhin). They are not very visible. Three orbits represent the colors of the Russian flag. 98 is the year of cosmonauts class (selection).
Maybe Fedor after his landing next night will give us more details about his personal patch.
