What was the first Soviet manned spaceflight that had a mission patch designed for it that was worn by the crew during the mission?

Was it the Apollo-Soyuz Test Project? Or another mission?

The very first space patch ever was worn by Valentina Tereshkova on her Wostok-6 mission in June 1963.

It was on the left shoulder of the sky blue flightsuit she was wearing underneath her pressure suit.

There is a photo of the blue internal suit here, as part of a photo tour of the Zvezda Museum.

Is there a photo or film showing Tereshkova wearing the suit with the patch visible?

The first patch worn on a RSS (regardless of whether it was "commissioned / designed" for the mission) was, as far as I know, Voshkod 2.

Yes, we found a movie clip just recently. Here is the entire item (relevant footage at 0:59 and 1:15).

Both the Tereshkova patch and the Leonov (Woskhod-2) patch (despite some authors claiming it was arranged by Leonov himself) came with the suits. Both were designed and sewn onto the suits at the Zvezda factory.

For Tereshkova, at least three suits (all with the patch) were prepared (we know this because it is on display in at least three different museums in Russia).

So, a search online (including SpacePatches.info and its Flickr account) does not seem to turn up a clean, detailed photo of Tereshkova's patch. Does someone have one to share?

Wonder if anyone has ever tried to render that irregular shape into some type of patch design?

Or, if anyone is game to TRY to... hint, hint...

It looks like an elongated (vertically) curved shield with protrusions for the bird, olive branch and "CCCP". Would be a tough job to make it into a patch like we are familiar with.

Zvezda and spacepatches.nl have cooperated to have a limited number of reproductions and souvenir versions of this historic patch available for the 50th anniversary of Tereshkova's mission. Stay tuned!

For this project, Zvezda agreed to remove Valentina's suit from the display, lay it out flat on a table, make photographs and supply us with the exact measurements. For obvious reasons, we can not share any pictures yet...
